Neat little poem. I like the theme behind. Hits a litle close to home, as I've seen people I love going through what the person being talked to is going through. Hm. Wish I could comment on the literary aspects, but I'm still not the best with poetry. I do like the simple, straightforward syntax. Makes it seem a lot more personal.
