I think Fallen suits this. The title could refer to so much that you've written about that I wouldn't change it. Images are strong and you are able to merge the crash (I take it it was a car crash) with the diseased world around the person. Very well done esp the last couple of lines contrasting the casting away of rubbish to the slipping of sand, eventually its all the same. I look forward to reading more of your stuff
