I like it! It is very well written and has a nice flow with the rhyming, and you utilized the topic of Bipolar Disorder well in your writing, stirring a sense of sorrow in the reader. However, as someone who suffers from the condition, I have to say that I have rarely realized my mood swings the moment they were happening. They do cause frustration, but for me it isn't something I monitor. Other people with the disorder may have symptoms closer to what you describe here, though.
