I agree with all of those-except the prologue. Sometimes prologues can actually be really helpful and intriguing intros to stories-and you neglected to mention the ones that start at the end of a story.
Other than that, I do agree with most of the points you're making, I was just a bit annoyed by your prologue paragraph.
