this is quite sad, although I know the feeling. I dont know if this is a hypothetical poem, but the answer to all the questions of your poem is: Yes!
It IS worth it to hold in every grain of life, no matter how challenging it is. The benifits are great in the end. The world is a cruel thing sometimes...and as much as it sucks to have to deal with it, we most often have to grin and bear it. Again the benifits are great in the end.

Sand and time, sand and life all indicating an definite end when the last grain falls. The goodbye at the end puncuates that.
