I've never seen this described in the way it has here. Powerful, heart-achingly so. The imagery is vivid. The idea of a body as a landscape isn't new one but using it to describe a diminishing form is a different approach. Also striking the motivation behind the condition. The first stanza is pretty much what one has come to expect. The idea that there's a lack of beauty that a person feels can only be achieved with the loss of weight. But they never truly mean to be as self-destructive as implied here.
The last line is hopeful however. And though, unexpressed, one knows that the reason behind their continued fight will hardly be a flimsy one.
A difficult subject. You've succeeded in putting heart to it. Well done.

This was interesting to read. The imagery is quite striking. The idea of a psychologist being a receptacle, a trash can. The feeling that writing is to bleed over a page. Life's blood has always been a powerful and valuable thing to pour out. It could speak both of how valued the writing can be and from a cutting pov the body gives release to what the mind contains. The pen in one's hand the same as a blade. Grim, but effective.

I love this piece. Dark and revolting ocd and yet your want to re-read the piece slowly again. Good write!
